Is Case Management Right for Me?
Case management is part of our team-based approach to care and is designed to work alongside therapy services. To receive case management, clients must be actively engaged in therapy with our practice so that services can be coordinated effectively and support consistent progress.
Case Management can include:
Connecting families to community resources (housing, financial, educational supports etc.)
Coordinating care between providers (schools, medical, dietary etc.)
Advocacy and guidance through complex systems
School challenges or IEP/504 support
Helping families follow through on treatment recommendations
Reducing barriers to mental health care
